Below is help completing the online document found in the email from ABC Funerals titled ‘Signature requested on’:

Note, the forms within the document are not formatted to be physically printed, they are optimized for online completion with ‘Adobe Sign’.


To start the process of completing the document, select “Review and sign” from the email

Fill in the documents as thoroughly as possible, paying special attention to page 2 “Information about the deceased”.

Where the form is asking for an initial or signature, click on the field that indicated “click here to sign” or “initial” and a new box will open, click the signature/ initial line and type in your signature/ initials then click “APPLY”. The box will close and your signature should be visible on the document


When all of the required fields have been completed, a new pop up will appear at the bottom of the page that says “CLICK TO SIGN” once you select that, we will each get an emailed copy of the completed document. A funeral director will also be in touch to confirm that the documents have been received.


If you do not see this pop-up, you may have to “accept cookies” at the bottom of the page.

If the pop-up still does not appear, go back through the document from the beginning to ensure all of the initial and signature fields are filled in.

    Page 1- Breakdown of fees


    The “Purchaser” must be the person who has the legal authority to be making the arrangements. If you are assisting the legal authority to fill in the forms, please ensure it is their information you are imputing and not your own.


    If you wish to add/ remove something from the contract, kindly proceed with the forms as-is and reply to the first email titled Cremation (1 of 2) with what you would like adjusted and the page number(s) in question, along with the best number to reach you at so that a Director familiar with your arrangments can quickly contact you during business hours to give effect to your desired changes.

    All of this information is very important and will be used to register the death with the Ontario government.


    Please fill it out as accurately as possible and double check that no errors have been made.


    We will be registering the death with the information exactly as you have provided it. If there are any errors, it will be your responsibility to have it corrected.


    If you do not know the signing doctor’s name, you do not have to provide this information, we can get it from the hospital


    “Last name or single name at birth” refers to their last name at the time of their birth.  Ie/ decease’s maiden name, mother’s maiden name, last name before a legal name change due to immigration, etc.


    “Last name or single name of deceased’s partner (before this marriage or relationship)” indicates the deceased’s most recent spouse’s last name before the marriage (ie/ maiden name or previous married name) or current last name if no name change occurred after the marriage.

    Visual Identification of Deceased: If the Purchaser does not wish to visually identify the Deceased and assumes all liability for mistaken identity relating to incorrect identification and waives any claim against the Transfer Service or its employees due to such lack of identification.


    This means that we will be identifying the deceased based on their hospital/ nursing home ID (concurrently with the description on page 5) or a family member, nursing home employee or authorised personnel will be identifying the deceased to us. If the deceased is misidentified to us, we are not liable.


    Possession of Cremated Remains: By initialing, the Purchaser authorizes the Transfer service to take possession of the cremated remains of the Recipient. The Transfer Service will retain the cremated remains for 2 weeks from the date the cremation took place. 


    This means you are allowing us to pick up the cremated remains from the crematorium on your behalf. We request that you make arrangements to pick up the cremated remains from us within 1 week of the cremation being complete. If you will require more time than that, kindly advise us before the cremation takes place. If you do not pick up the cremated remains within 1 year, they will be interred without notice, at your cost.

    Legal authority: The Purchaser declares that he/she is legally authorized to make, or charged with the responsibility for, the arrangements contained in this contract.


    You are confirming that you have the legal authority to make these arrangements or you have written permission from the legal authority.


    Price list and contract Consumer Information Guide: The Purchaser acknowledges having received copies of the Transfer Service’s price list, a fully executed copy of this contract and the Consumer Information Guide.

    The description assists us in identifying the deceased. It does not have to be exact.


     Preparation vs. no preparation is completely up to you. If you plan on seeing the deceased, we recommend preparation, but it is not necessary. If you will not be seeing the deceased there is no reason to have any preparation unless you feel strongly about it.


    We do not offer embalming. Our basic preparation includes:


    Dressing ($200) you provide the clothing

    Setting Features ($150) (closing the eyes and mouth for a natural appearance)

    Bathing ($150)

    $450 total if you select all 3

    This outlines your rights to cancel.


    Essentially, you can cancel at any time for a full refund.


    If any services have been provided in part or in full, you are responsible for those items only.
